PREAMBLE FOR INVESTIGATIVE PROCEDURES

You will be asked to participate in a questionnaire study that examines your thoughts and attitudes about past hurricanes and communication during disasters. There are no right answers to the questions you will be asked. Our interest is solely in your thoughts and attitudes. Upon completion of the survey, participants can self-enter into a raffle to receive one (1) gift card valued at $75.
There is minimal risk of discomfort or personal time inconvenience to you. In particular, the anticipated discomfort in response to the questions that will be asked should be no greater than that associated with everyday conversations. The entire study should take you approximately 20 minutes to complete, during which time, your participation is completely voluntary. You are free to withdraw your consent and discontinue participation in this project at any time without affecting either your right to receive ongoing care from or your relationship with Howard University. You will be kept anonymous during all phases of this study including any experimental writings, published or not.
The Howard University Institutional Review Board, as well as all investigators, will have access to the records of this project. If you have any questions that you would like to discuss with someone other than the investigators on this project, you are free to contact the Howard University Institutional Review Board, at (202) 865-8597 between 8:30 a.m. and 5:00 p.m., Monday through Friday.
